Checklist for Admissions
- COMPLETE THE APPLICATION
The College of Menominee Nation (CMN) offers several methods to apply. Please select the option that works best for you.
- Download an application and submit the completed application to the following address:

- Contact the College to obtain a paper application by calling 715-799-6226 or toll free at 800-567-2344. Student Services extension is 3051.
- REQUEST TRANSCRIPTS
CMN requires official transcripts from your high school, or GED/HSED location. If applicable, official transcripts from post secondary institutions are required. High school, GED, or HSED transcripts may be waived if student has successfully completed 12 or more college credits.

- PROVIDE DEMOGRAPHIC INFORMATION
Complete and submit the Demographic Verification form. If you are American Indian, you may elect to submit a copy of your Tribal Enrollment Card in place of this form.
- SCHEDULE AN ACCUPLACER TEST
This assessment is used to determine English and math skill levels, and allows for appropriate course placement. Tests are administered at both campus locations, and there is no fee if applying to CMN. Call to schedule an appointment at 715-799-6226. If you choose to schedule your appointment in Green Bay, ask for ext. 4000, and in Keshena ext. 3169. What is the Accuplacer Placement Test?
When the Admissions office has received and processed all of the required information, you will receive a Letter of Acceptance. Once you receive your letter, the next step will be to schedule an appointment with an advisor to register for classes.
