Nursing Assistant - How to Apply

 

Steps to entering the Nursing Assistant Program

  • Submit completed Nursing Assistant Application packet including HPOG grant application and previous year’s signed US tax return. (Nursing Career Ladder (HPOG) Grant approved participants get priority in enrollment).

    - Wisconsin Caregivers Background Check Form

  • Complete necessary grant requirements:This includes support and academic intakes, sign a Student Success Plan outlining Nursing Assistant course approval.
  •  

  • If you are ineligible for Nursing Career Ladder funding and there is a space available in the class, it is your responsibility to provide academic readiness documentation by providing either Accuplacer or TABE test scores. In addition to being responsible for purchasing your own textbooks, scrubs, gait belt. You will also be responsible to pay for your State certification exam.

  • Review Nursing Assistant Functional Abilities Criteria and sign the Nursing Assistant Functional Abilities Statement of Understanding. If you are currently under the care of a physician, they must also sign that you are able to meet the functional abilities.

  • You will need to get a 2-step TB test prior to enrollment, if a positive is read; a chest x-ray will also be required.

  • To register for the HLT-095 Nursing Assistant course, you must have appropriate placement scores and consent of the Nursing Department.
